When a Death Occurs
If death has occurred at home, nursing home or hospice and the coroner is not involved, we will arrange for our Mortuary Care Team to be present at site within 2 hours. In this case, GP will confirm and issue Medical Certificate Cause of Death (MCCD). If death has occurred in hospital, we will assist you to complete:
- A Transfer Authority form (deceased discharge form) needs to be completed and signed.
- Registrar to confirm that an MCCD and the Attending Cremation Permit are competed and available.
- Coroner’s Court to fax through the order of disposal and a *Cremation Permit (*if cremation is chosen).
- The transfer authority and at least one set of these documents must be completed before collection of the deceased from the hospital can be arranged.
In the case of a cremation, we require a Statutory Declaration authorising the funeral director and the crematorium to proceed with the cremation process, signed by either the Next of Kin or nominated executer.

In the case of a cremation
Where possible the cremation will take place within 3-5 days of receiving all relevant paperwork. If the cremation is required to take place sooner please let us know.
The cremation will take place at the crematorium of our choice. Additional charges will apply if you choose to use a different crematorium.
Your loved one’s ashes will be available for collection from our office 4 working days after the committal. We can return the ashes to you by hand delivery (local area only, on a mutually acceptable day) within 28 days of the cremation or via Registered Australia Post. We do not use couriers.
In the case of a burial
Where possible the burial will take place within 3-5 days of receiving all relevant paperwork. If the burial is required to take place sooner please let us know.
The burial will usually take place at the cemetery of your choice.
